How do I make changes to my existing coverage?

If you would like to make a change to an existing policy that you have with Aflac or update your personal information, please visit Online Services for Policyholders.

How do I file a claim?

Aflac offers various options for filing a claim.

1.  Start the claim process online by logging into Online Services for Policyholders.  This process allows the submission of a claim electronically, by completing the claim form online and uploading any supporting documents.

2. Download a claim form online at aflac.com.  By selecting the appropriate claim form, you will have the option to download the pdf version using Adobe Acrobat Reader. 

3. Contact an Aflac customer representative to request a claim form by calling Aflac at 1.800.992.3522. Upon request, the applicable claim forms will be mailed to the policyholder.

4. Your Aflac agent is always available.  Please contact your Aflac agent for any additional assistance with the claim filing process.
